

The Need
Located in Southwest Michigan, Allegan County is known for its farmland, forests, and strong sense of community. While the area offers natural beauty and deep local connections, its rural nature also creates challenges- especially when it comes to accessing health care.

Many residents face barriers such as transportation, limited providers, cost, insurance gaps, and language differences. These challenges contribute to ongoing health disparities and unmet community needs. Our services are designed to remove those barriers, address local health disparities, and meet people where they are- because accessible, community- centered care matters.
Our Response
Grounded Community Health provides accessible, compassionate physical care for uninsured and underinsured adults in Allegan County. Our approach is designed to meet community needs by:
